After contacting our office, you will receive an email with your unique username.
In the email, click “Set Up Portal Account” or click “Portal Link.”
Complete initial login to authenticate your identity.
Click on the “Questionnaire” tab and complete the three questionnaires.
Complete all the questionnaires listed prior to your visit
Complete the New Patient Form and bring this to your initial visit.
